About the Program

We know that financial hardship creates obstacles on the path to building a stable life—but we also believe that everyone, regardless of financial status, should have easy access to their personal records.

With the Birth Certificate and ID Program, Second Chance Society assists clients with obtaining the vital documentation they need to secure a job or receive job training. Whether you need a birth certificate or a Florida state ID, Second Chance Society can help you take that next step toward a stable career and a more secure future.

Eligibility

In order to qualify for Birth Certificate or ID assistance, a case manager must confirm that your request is directly related to your job or vocational training.

If eligible, you and your case worker must submit an application, ensuring that all fields are completed and a copy of your completed state application is attached as requested. All applicants must also be:

  • Born in the US or a US territory
  • 18+ years of age. If you are under 18 and need assistance, click here.
  • Homeless and/or low-income

Take the Next Steps

Application and Follow-Up

Once your case manager has submitted the Birth Certificate/ID Program Application, Second Chance Society Staff will review it and reach out within 5-7 business days with more information. If your request is approved, checks can be picked up at our offices or will be mailed directly to your case manager within 7-10 days of application submission.
