The Case Manager has worked with the client to identify, complete, and print the appropriate birth certificate application, including details of the payee and the total amount requested.

The Case Manager has confirmed that the request is employment or vocational training-related.
Get Assistance with an ID or Birth Certificate
To receive funding for a Birth Certificate or Florida ID Card, the Case Manager must complete the “BC/ID Program Application” LINKED BELOW. Once successfully submitted, SCS staff will review the request and reach out within 3-5 business days to discuss the next steps. If approved, checks can be picked up from the SCS office or, if needed, will be mailed directly to the referring Case Manager within 7-10 business days of application submission.
